Phasezerker for guns that require good aim and crits, Driver for stuff where bullets home in or where aiming properly isn't that important. Cutsman/Tediore are godly with the Driver for example, but if I run with an Unforgiven where I need to land headshots, I prefer Phasezerker.
Phasezerker is just a good all-around damage mod. Kensei is good if you're fakegrasping all the time and spec'd into a lot of the melee skills, as it's one of the few effects in the game that you can still trigger from a fakegrasp.
